Job summary

Alpine Care Home Health seeks an experienced Licensed Practical Nurse to deliver direct, compassionate in‑home care under RN supervision. You will support care plans, perform skilled nursing tasks, and ensure high quality care aligned with patient needs across their homes.

Responsibilities include intermittent skilled visits, documentation, and collaboration with physicians, supervisors, and the care team. A valid LPN license and a current CPR certification are required.

Qualifications

• Current, unencumbered LPN license in the state of practice.
• Valid driver’s license and good driving record.
• Ability to exercise sound clinical judgment and create care plans.
• Strong communication skills and comfort with documentation.
• CPR certification and basic computer skills.

Responsibilities

• Provide intermittent skilled nursing visits per the patient Plan of Care.
• Support the care plan and perform nursing tasks in home settings.
• Notify physician/Clinical Supervisor of significant findings.
• Maintain 90% productivity and participate in PI activities.
• Communicate with the care team and participate in discharge planning.
