Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• Home care experience preferred; able to manage multiple tasks and work independently.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours as required to meet client needs.

Responsibilities

• Provides high quality clinical services within the scope of practice and coordination with health care team.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and reports patient needs and changes in condition.
• Functions in diverse home care settings and with diverse patient populations.
