Job summary

Yale NewHaven Health is seeking a Licensed Practical Nurse for home health visits in Connecticut. You will provide nursing care under an RN, educate patients and caregivers, and document progress and changes in condition.

The role requires CT LPN licensure (or eligibility), CPR certification, a valid driver’s license with insurance, and at least 1 year in acute care. Travel to multiple stops daily is expected.

Qualifications

• Graduated from an approved Practical Nurses Education Program.
• Connecticut LPN license (or eligible).
• Valid driver's license and proof of car insurance when using a personal vehicle.
• CPR certification required or must be obtained within 3 months (90 days) of hire.
• Minimum one year experience in acute care; home care is a plus.

Responsibilities

• Assess patients within scope of practice at every home visit.
• Carry out treatments per MD orders; report to RN case manager and/or clinical manager.
• Report all changes in condition to MD.
• Provide patient/caregiver education as per care plan.
• Complete all documentation within 24 hours with changes in condition and follow-up with provider.
• Attend all staff meetings and required trainings (inservices).
• Comply with mandatory requirements including Healthstreams.
• Follow policies and procedures.

EXPERIENCE

Minimum one year experience in the acute care setting. Home care experience preferred, but not required.

LICENSURE

License Practical Nurse with valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.

PHYSICAL DEMAND

Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Requires exposure to blood and body fluids, hazardous materials, and communicable diseases.

Additional Information

Must have own transportation with valid driver's license and insuranceMust be able to work independently and as part of a team. Teamwork is crucial to this role. Compliance with professional requirements (licensure, training courses, BLS, fit testing, documentation, etc.) is required at all times.
