Job summary

Yale NewHaven Health is seeking a Licensed Practical Nurse to provide home health nursing under the direction of an RN. You will assess patients during home visits, administer treatments per MD orders, and educate patients and caregivers as outlined in the care plan.

You will ensure timely documentation, collaborate with the care team, and maintain CPR certification and CT licensure. Valid driver's license and reliable vehicle are required for travel between stops across CT.

Qualifications

• Graduated from an approved Practical Nurses Education Program.
• Minimum one year experience in acute care; home health preferred.
• Connecticut LPN license required; driver's license and car insurance when using personal vehicle.
• CPR certification must be maintained.

Responsibilities

• Assess patients during home visits within scope of practice.
• Carry out treatments per physician orders and report to RN case manager.
• Report condition changes to physician.
• Educate patients and caregivers per care plan.
• Complete documentation within 24 hours with updates.
• Attend staff meetings and required trainings.
• Comply with mandatory requirements, including Healthstreams.
• Follow policies and procedures.

The Licensed Practical Nurse works under the direction of a registered nurse to provide nursing care, to assist the patient in learning self-care techniques and to prepare clinical and progress notes.

Responsibilities

• 1.Assesses patients within scope of practice at every home visit.
• 2.Carries out treatments per MD orders. Reports off to RN case manager and/or clinical manager.
• 3.Reports all changes in condition to MD.
• 4.Provides patient/caregiver education as per care plan.
• 5.Completes all documentation within 24 hours. Must include changes in condition, follow-up with provider or other disciplines with supporting documentation.
• 6.Attends all staff meetings, inservices, and all other meetings as required.
• 7.Compliance with all mandatory requirements -inclusive of Healthstreams.
• 8.Follows policies and procedures.

Qualifications

EDUCATION

Graduated from an approved Practical Nurses Education Program

EXPERIENCE

Minimum one year experience in the acute care setting. Home care experience preferred, but not required.

LICENSURE

License Practical Nurse with valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.

SPECIAL SKILLS

Excellent verbal and written skills. Excellent clinical knowledge. Strong computer skills.

PHYSICAL DEMAND

Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Requires exposure to blood and body fluids, hazardous materials, and communicable diseases.
