Job summary

Holland Hospital is seeking a Home Health LPN to provide skilled nursing care and comprehensive case management in patients’ homes. You will conduct holistic assessments, coordinate services with the interdisciplinary team, and monitor progress to support safe transitions of care.

The role requires an accredited LPN program graduate with Michigan licensure, BLS, and strong clinical and communication skills.

Qualifications

• Graduate of accredited LPN program.
• Current Michigan LPN License.
• BL S certification within 60 days of hire.
• Provide compassionate care.
• Administer medications and treatments as prescribed by physicians.

Responsibilities

• Conduct holistic assessments and develop individualized plans of care.
• Coordinate services across the interdisciplinary team.
• Monitor patient progress throughout the episode of care.
• Document accurate and detailed patient information.
• Educate patients and families about condition management and care.
• Collaborate with physicians, RNs, therapists and social workers.
• Ensure compliance with regulations and safety standards.
• Perform wound care and routine nursing duties as prescribed.
