Adoration Health in Albemarle, NC is seeking a dedicated Home Health LPN to deliver patient-centered, 1:1 care in home settings. You will follow each patient’s plan of care, monitor status, and document outcomes to support physicians and families.

This full-time role offers flexible scheduling, strong team support, and opportunities for advancement within a growing company.

A valid LPN/LPT/LVN license in NC and CPR certification are required; home health experience preferred. Driving is required.

Qualifications

• Active LPN/LPT/LVN license with unrestricted registration
• CPR certification
• 1+ years of nursing experience, home health preferred
• Valid driver's license with proof of insurance

Responsabilités

• Provide 1:1 home health nursing per plan of care
• Administer medications, feedings, oxygen as prescribed
• Document nursing actions and communicate with family and physicians
• Coordinate supplies and ensure patient needs are met per physician guidance
