Job summary

Adoration Health in Albemarle, NC is hiring a Home Health LPN to join our patient-centered team. This role provides 1:1 care in patients' homes, following each plan of care and maintaining dignity and independence.

Qualifications

• LPN/LPT/LVN degree with current unrestricted state license
• Valid driver’s license, driving record and proof of insurance
• CPR certification
• 1+ years nursing experience preferred
• Home Health care experience preferred

Responsibilities

• Providing 1:1 care for client per plan of care while maintaining dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed and assess results
• Documenting nursing actions and communicating with family, case manager, and physicians; report changes in condition
• Coordinating or ordering supplies, medications, and feedings to ensure adequate stock
• Ensuring patient needs are met in accordance with physician instructions
