Aveanna Healthcare is seeking a Visiting Licensed Practical Nurse (LPN) for our VNA Certified division in Massachusetts. The role delivers skilled nursing, therapy support, and home health aide services in patients' homes for diverse populations across ages with chronic or acute needs.

Qualifications

• Active nursing license in the state of practice.
• Current CPR certification (with hands-on component).
• TB test current within last 12 months.
• Driver's license and reliable transportation.

Responsibilities

• Deliver quality patient care in the home per physician orders under RN supervision.
• Document and communicate with physicians and other caregivers.
• Identify changes in patient needs and adjust care to manage risk.
• Educate patients and families based on individual needs.
• Provide home-based care using a range of nursing skills (wound care, injections, IVs, etc.).

Our Visiting Licensed Practical Nurse for our VNA Certified division provides short- and long-term home care services for patients covered by Medicare, Medicaid, Private Insurance, and Private Pay. Services include Skilled Nursing, Physical Therapy, Occupational Therapy, and Home Health Aide support delivered in patients’ homes. We serve diverse populations across all ages, including patients with chronic conditions, behavioral health needs, skilled nursing requirements, and therapy needs.

Pay

$30-38 per hour/visit based on the pay point model

What You'll Do

• Deliver quality patient care in compliance with physician orders under direction of their Registered Nurse teammate.
• Perform the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers.
• Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.
• Educate clients and their family members based on client’s specific needs.
• Provide care in patients home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes etc.

Qualifications

• An active Nursing License in the state of application
• Current CPR certification (with hands-on component) or able to obtain one
• Current TB test
• Must have drivers license, reliable transportation,and auto insurance

Preferred

• Medicare Skilled Nursing experience
• 1-year LPN experience in a health care setting
