Job summary

Aveanna Healthcare is seeking a Licensed Practical Nurse to provide and document skilled nursing care in patients' homes under the supervision of a Registered Nurse.

This full-time position serves territories including Hartland, Sussex, Pewaukee, Waukesha, and Oconomowoc with a Monday–Friday schedule.

You will implement care plans, perform hands-on nursing tasks, educate families, and coordinate with physicians while maintaining confidentiality and professional boundaries.

Qualifications

• Active LPN license in the state of application.
• Valid CPR, Driver's License and Proof of automobile insurance.
• 1 year LPN experience in home health or healthcare setting preferred.
• Home Care Home Base experience a plus.

Responsibilities

• Deliver quality patient care under the direction of a Registered Nurse.
• Document care and communicate with the patient's physician and other caregivers.
• Recognize changes in patient needs and implement care to prevent or reduce risk.
• Educate clients and their family members based on client needs.
• Provide home health care including wound care, infusions, catheter care, and related procedures.

Position Overview

The Licensed Practical Nurse Full Time is an essential part of the team responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians’ orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

Schedule: Monday - Friday

Territory: Hartland, Sussex, Pewaukee, Waukesha, Oconomowoc

Essential Job Functions

• Deliver quality patient care in compliance with physician orders under direction of their Registered Nurse teammate.
• Perfom the continuum of hands on client care through documentation and timely communication with the client's physician and other caregivers
• Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.
• Educate clients and their family members based on client’s specific needs.
• Provide care in patients home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes etc.

Requirements

• An active LPN License in the state of application
• Valid CPR, Drivers License and Proof of Automobile insurance

Preferred

• 1-year LPN experience in a home health or health care setting
• Home Care Home Base exp a plus
