Job summary

Aveanna Healthcare is seeking a Licensed Practical Nurse to provide and document skilled nursing care in patients' homes in the Sioux City area. This full-time role follows a daytime, Monday–Friday schedule with travel to Sioux City and surrounding communities.

You will work under a Registered Nurse, performing hands-on care, educating patients and families, and coordinating with physicians. Active LPN licensure, CPR, and a valid driver's license with insurance are required.

Qualifications

• Active LPN license in the state of application.
• Valid CPR certification.
• Valid driver's license and proof of automobile insurance.

Responsibilities

• Deliver quality patient care in compliance with physician orders under the direction of the Registered Nurse teammate.
• Perform the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers.
• Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk.
• Educate clients and their family members based on client’s specific needs.
• Provide care in patients' home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes.

Position Overview

The Licensed Practical Nurse is an essential part of the team responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians’ orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

Full-Time visits needed Monday - Friday daytime hours for Sioux City and surrounding areas

Essential Job Functions

• 1-year LPN experience in a home health or health care setting
• Home Care Home Base exp a plus
