Job summary

Aveanna Healthcare in Walker County, Alabama is seeking a Licensed Practical Nurse (LPN) to join our home health team on a full-time basis. You will deliver skilled nursing care per the care plan and physician orders, documenting care and communicating with the supervising Registered Nurse.

Schedule is Monday through Friday with a focus on patient education, wound and infusion care, and coordination with families and physicians to maintain high-quality, confidential treatment in the patient's

Qualifications

• Active LPN license in the state of application.
• Valid CPR certification.
• Driver's license in good standing.

Responsibilities

• Deliver quality patient care in accordance with physician orders under the supervision of a Registered Nurse.
• Maintain continuity of hands-on client care through thorough documentation and timely communications with the client’s physician and other caregivers.
• Identify changes in patient needs and implement interventions to prevent or mitigate risk.
• Educate clients and their families based on specific care requirements.
• Provide in-home care using skills such as phone triage, wound care, infusions, catheter care, and PICC line dressing changes.

The Licensed Practical Nurse Full Time is an essential part of the team responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians’ orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

Schedule: Monday - Friday

Territory: Walker County

Essential Job Functions

• Deliver quality patient care in compliance with physician orders under direction of their Registered Nurse teammate.
• Perfom the continuum of hands on client care through documentation and timely communication with the client's physician and other caregivers
• Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.
• Educate clients and their family members based on client’s specific needs.
• Provide care in patients home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes etc.

Requirements

• An active LPN License in the state of application
• Valid CPR and Drivers License

Preferred

• Medicare Skilled Nursing experience
• Basic understanding of Oasis
• 1-year LPN experience in a home health or health care setting
• Home Care Home Base exp a plus
