Job summary

Concierge Home Care is seeking a Home Health LPN to deliver skilled nursing care in patients’ homes across Heathrow, FL. This full-time role offers flexibility, autonomy, and 1:1 patient care with no hospital shifts.

You will administer medications, perform assessments, and document care using AI-assisted tools in WellSky/Kinnser EMR. Benefits include health insurance after 30 days, PTO from day one, quarterly bonuses, and referral incentives.

Qualifications

• Active, unencumbered Florida Licensed Practical Nurse (LPN) license
• Valid driver’s license, reliable transportation, and auto insurance
• CPR certification

Responsibilities

• Provide skilled nursing care under the supervision of a Registered Nurse
• Perform patient assessments and assist with care planning
• Administer medications and treatments as ordered
• Educate patients and caregivers on disease management and care plans
• Monitor patient condition and report changes to the supervising RN
• Collaborate with physicians and interdisciplinary teams
• Complete documentation using streamlined, AI-assisted tools
