Job summary

Concierge Home Care in Tallahassee, FL is seeking a Home Health LPN for flexible, autonomous, one-on-one patient care in the home. You will manage your own schedule with options including PRN or modified full-time, covering Tallahassee/ Leon County.

The role includes 1:1 patient care, medication administration, patient education, and collaborative care planning with an RN. Benefits include health insurance starts after 30 days, PTO accrues from day one, quarterly bonuses, free CEUs, and an EAP

Qualifications

• Active, unencumbered Florida LPN license.
• CPR certification is required.
• Home health experience preferred but not mandatory.

Responsibilities

• Educate patients and caregivers on disease management and care plans.

Your Role as a Home Health LPN

• Provide skilled nursing care under the supervision of a Registered Nurse

Qualifications
