This role focuses on helping patients recover at home with personalized nursing support. Requirements include graduation from an accredited practical nursing program, active VA LPN license, CPR and TB testing, and at least 1 year of home health experience.

Qualifications

• Graduate of an accredited practical nursing program.
• Active LPN license in Virginia.
• Minimum of 1 year of LPN experience (home health preferred) or completion of office preceptor program.
• CPR certification and TB skin test required.
• Knowledge of state and federal home health regulations.
• Strong clinical judgment, communication, and critical thinking skills.
• Compassionate, dependable, and patient-focused.
• Ability to work independently in a home setting.

Responsibilities

• Provide home-based nursing care to patients recovering from illness, injury, or chronic conditions under the supervision of an RN.
• Monitor patient condition, obtain vital signs, and perform routine nursing assessments.
• Document patient progress and report changes in condition to RN supervisor.
• Assist with implementing individualized patient care plans.
• Educate patients and families on medications, treatment plans, self-care, and home safety.
• Collaborate with physicians, RNs, CNAs, aides, and interdisciplinary team members to coordinate patient care.
• Assist with discharge planning and ensure continuity of care.
• Maintain accurate and timely documentation.

We are currently seeking compassionate and skilled Home Health Licensed Practical Nurses - LPN to provide personalized, one-on-one patient care throughout Collinsville and the surrounding coverage areas.
