Job summary

Beacon Home Health in Jackson, TN seeks an experienced Licensed Practical Nurse (LPN) to join our home health team. The LPN will deliver quality patient care in patients’ homes, in coordination with physicians and RNs.

Responsibilities include teaching families, monitoring patient responses, and documenting changes. A valid Tennessee LPN license and CPR certification are required, along with reliable transportation for daily travel to multiple residences.

Qualifications

• One year of LPN clinical experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification.
• Driver's license, vehicle insurance, and access to reliable transportation.
• Must be able to commute to multiple patient residences daily within the service area.

Responsibilities

• Provide skilled nursing care in patients' homes per plan of care.
• Instruct patient families on care requirements and home safety.
• Monitor and report patient status to supervising nurse/physician.
• Coordinate care with physicians and RNs; do not alter the plan of care.
