Job summary

Adecco is seeking a Licensed Practical Nurse (LPN) for home health in Grants Pass, Oregon. You will provide skilled nursing care in patients' homes, build trust with families, and work independently with support from an experienced clinical team.

This full-time role requires 1 year of clinical nursing and 1 year of home health experience, Oregon LPN license, CPR, and valid driver's license. Expect growth opportunities and a focus on patient comfort and safety.

Qualifications

• At least 1 year of clinical nursing experience.
• At least 1 year of home health experience.
• Graduate of an accredited practical nursing or vocational nursing program.
• Active and unrestricted Oregon LPN license.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to travel between patient homes throughout the assigned territory.
• Strong communication, organization, and clinical documentation skills.
• Comfortable working independently and managing multiple patient needs.

Responsibilities

• Conducting in-home nursing visits based on established care plans and physician orders.
• Monitoring patient progress and identifying changes that may require additional attention.
• Providing medications, treatments, and other prescribed nursing interventions.
• Teaching patients and caregivers about medications, treatment plans, health conditions, and ongoing care.
• Maintaining accurate and timely patient records and clinical documentation.
• Communicating with physicians, therapists, case managers, and other healthcare professionals.
• Helping patients achieve the best possible outcomes while maintaining their comfort, safety, and dignity.
• Responding to patient needs as part of an established on-call rotation.

We are currently seeking a compassionate and dependable LPN to provide skilled nursing care to patients throughout the Grants Pass area. This is a great opportunity for a nurse who enjoys building relationships with patients, working independently, and providing personalized care outside of a traditional clinical setting.

You’ll be supported by an experienced healthcare team while having the independence to manage your visits and deliver care where patients need it most — in their own homes.

• Full-Time position
• Monday–Friday, 8:00 AM–5:00 PM

The position is based out of Grants Pass, Oregon, with patient visits throughout the surrounding communities:

A valid driver’s license, reliable transportation, and the ability to travel throughout the service area are required.
