Job summary

Adecco in Medford, OR is seeking an experienced Licensed Practical Nurse (LPN) for Home Health visits in the surrounding communities. This full-time role offers independence to manage patient visits, while supported by a clinical team to ensure quality care.

You will conduct in-home nursing visits, administer medications, teach patients and caregivers, document care, and collaborate with physicians and case managers. Travel throughout the service area is required and on-call rotations apply.

Qualifications

  • At least 1 year of clinical nursing experience.
  • At least 1 year of home health experience.
  • Graduate of an accredited practical nursing or vocational nursing program.
  • Active and unrestricted Oregon LPN license.
  • Current CPR certification.
  • Valid driver's license and reliable transportation.
  • Ability to travel between patient homes throughout the assigned territory.
  • Strong communication, organization, and clinical documentation skills.
  • Comfortable working independently and managing multiple patient needs.

Responsibilities

  • Conduct in-home nursing visits based on established care plans and physician orders.
  • Monitor patient progress and identify changes requiring additional attention.
  • Provide medications, treatments, and other prescribed nursing interventions.
  • Teach patients and caregivers about medications, treatment plans, health conditions, and ongoing care.

  • Communicate with physicians, therapists, case managers, and other healthcare professionals.
  • Help patients achieve the best possible outcomes while maintaining comfort, safety, and dignity.
  • Respond to patient needs as part of an established on-call rotation.

Are you an experienced Licensed Practical Nurse (LPN) ready for a role where your nursing expertise directly impacts the lives of patients and their families?

We are currently seeking a compassionate and dependable LPN to provide skilled nursing care to patients throughout the Medford area. This is a great opportunity for a nurse who enjoys building relationships with patients, working independently, and providing personalized care outside of a traditional clinical setting.

You'll be supported by an experienced healthcare team while having the independence to manage your visits and deliver care where patients need it most — in their own homes.

  • Full-Time position
  • Monday–Friday, 8:00 AM–5:00 PM

The position is based out of Medford, Oregon, with patient visits throughout the surrounding communities:

A valid driver's license, reliable transportation, and the ability to travel throughout the service area are required.
