Job summary

UW Health in northern Illinois is seeking a registered nurse to join our day shift team at the Charles St. Clinic in Rockford.

The role requires an RN license, BSN preferred, and at least one year of acute care nursing experience; on-site care and discharge planning experience is valued.

Responsibilities

• Provide direct patient care as an RN in an acute care setting.
• Assist with hospital discharge planning and coordination of care.

Work Schedule:

• 100% FTE, full time
• Monday-Friday / 8:00am-5:00pm
• Every 3rd Saturday rotation
• You will work at UW Health Charles St. Clinic 2550 Charles St

Additional components of compensation may include:

• Evening, night, and weekend shift differential
• Overtime

Qualifications

• Graduate of an accredited School of Nursing/College. Required
• BSN. Preferred

Work Experience

• At least one year professional nurse experience within the last five years in an acute care setting. Required
• One year of experience in home care and hospital discharge planning. Preferred

Licenses & Certifications

• Valid RN license. Required
• Valid State driver's license. Required
