Responsibilities

• Conduct utilization review to ensure medical necessity and optimize reimbursement.
• Support intake, coordinate referrals, and ensure order accuracy.
• Build strong relationships with hospitals, physicians, and community partners to drive census growth and smooth patient transitions to home health services.
