AdvisaCare is seeking a compassionate Home Health RN to deliver skilled nursing services in patients' homes across Petoskey and surrounding areas. A licensed practical nurse (LPN) with a Michigan license is preferred, with at least 1 year of home health and hospice experience.

You will assess, plan, and revise patient care, supervise paraprofessionals, and participate in IDG conferences to ensure high-quality care and family support.

Formación

• Licensed Practical Nurse (LPN) with Michigan license in good standing.
• Minimum 1 year home health and hospice experience.
• Empathetic approach to patients and caregivers.
• Willingness to participate in field visits daily and team meetings.

Responsabilidades

• Provide skilled nursing care to patients in their homes.
• Supervise paraprofessionals delivering services.
• Conduct supervisory visits and clinical observations.
• Participate in IDG conferences and care-plan development.
