Job summary

The role requires state licensure, a BSN, and a minimum of one year of nursing experience; BLS is required and COS-C certification is preferred depending on location.

Qualifications

• RN license in state of practice (or multi-state license)
• Minimum 1 year of professional nursing experience preferred
• BLS required; COS-C preferred depending on location

Responsibilities

• Apply the nursing process (assessment, diagnosis, planning, implementation, evaluation) in the home health setting
• Coordinate care with patients, families and inter-professional teams
• Educate patients and family on health problems and self-management
• Triage and manage patient needs; ensure continuity of care
• Maintain compliance with Nurse Practice Act and state licensure requirements
