Sanford Health is seeking a field-based Home Health Registered Nurse to deliver direct patient care in patients’ homes across the community. You will manage a personal caseload, perform skilled nursing visits, and coordinate care with an interdisciplinary team, supported by leadership and travel reimbursement.

Qualifications

• Bachelor's degree in Nursing strongly preferred.
• Graduate from an accredited nursing program.
• Minimum of two years’ clinical experience preferred.
• Currently holds an unencumbered RN license where practice occurs.

• Provide direct patient care in patients' homes.
• Manage your own patient caseload and complete skilled nursing visits.
• Perform assessments, patient education, care coordination and documentation.
