Job summary

Stonerise Home Health and Hospice in Ohio seeks a dedicated Hospice Aide to observe and care for patients in their homes. You will provide personal care, assist with bathing, dressing, meals, and mobility, and report changes to the RN.

Qualifications

• Personal care for patients at home, including bathing and dressing.
• Taking vitals and monitoring patient conditions.
• Providing mobility assistance and activity facilitation.
• Documenting observations and reporting changes to RN.

Responsibilities

• Observe, report and document patient status and care performed.
• Provide personal care including bathing, dressing and feeding.
• Assist with ambulation, range of motion and daily activities.
• Report significant changes to the supervising nurse promptly.
