Position: Home Health - Full-Time - Per Visit, Licensed Vocational Nurse (LVN)

Position Details:

Our full-time LVN will perform a variety of duties to support the company's operations and ensure continuity of care. The candidate must maintain 36 productivity points weekly, participate in the compensated on-call program, and see all visits within their scope of practice. They will also assist in the onboarding of new hires. Responsibilities include administering skilled nursing care for patients of all ages in their homes, coordinating care with the interdisciplinary team, patients, families, and referring agencies, and participating in care coordination activities.

Job Responsibilities:

1. Be available Monday through Friday to accept patient visits.
2. Perform skilled nursing services under supervision of a staff LVN.
3. Observe, record, and report on patients' physical and mental conditions.
4. Administer prescribed medications and treatments as permitted by state regulations.
5. Assist physicians and staff LVNs in specialized procedures.
6. Help patients with activities of daily living and promote self-care.
7. Prepare and update clinical and progress notes weekly.
8. Report significant changes in patient conditions to physicians and relevant personnel.
9. Perform complex wound care if properly trained and documented.
10. Conduct routine venipuncture for labs if competency is documented.
11. Follow physician orders within LVN scope and home health standards; do not alter the care plan.
12. Participate in in-service training (minimum two per year), nursing reports, staff conferences, and QAPI initiatives.

Job Requirements:

1. Active state license as a Licensed Vocational Nurse or multi-state license via Nurse Licensure Compact (NLC).
2. 2-3 years of LVN experience.
3. Competent nursing skills adhering to accepted standards of practice; good interpersonal skills.
4. Current CPR, TB test, auto insurance, and COVID-19 vaccination proof.
