Job summary

BayCare Health System is seeking a Home Health Field RN in Sarasota to provide in-home patient care using nursing processes and clinical standards. A hands-on role overseeing a multi-disciplinary team, with visits, documentation, and adherence to physician orders.

Requirements include Florida RN licensure, BLS, and a valid FL driver's license within 30 days. The position offers a stable full-time schedule with occasional weekends and on-call duties.

Qualifications

• RN license is required.
• BLS (Basic Life Support) certification required.
• Driver's License State of Florida (FL license) required within 30 days of hire.
• Education: Associate's or Diploma in Nursing required; Bachelor's preferred.

Responsibilities

• Oversee the activities of the multi-disciplinary team that provides care to the patient.
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines.
• Ensure that all visits scheduled and provided are authorized and covered by physician orders.
• Review of patient visits weekly and provide plan to Patient Care Supervisor and team assistant, including anticipated weekend visits.
• Inform the scheduler of schedule changes and communicate caseload and patient care issues to Home Care Supervisor.

The Home Health Field, RN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols.

Required Certifications and Licensures

• Required 1 year Registered Nurse
