BayCare HomeCare seeks a dedicated Home Health Field RN in Sarasota to deliver patient‑centered care in patients' homes. The role supports a flexible schedule with potential weekend work, allowing a strong work‑life balance while ensuring high‑quality nursing care.

You will lead a multidisciplinary team, monitor therapeutic services, and ensure visits align with physician orders. A Florida RN license and BLS are required, with at least one year of home health experience preferred.

Qualificações

• BLS certification is required.
• RN license is required.
• Florida driver's license is required within 30 days of hire.
• Associate's or diploma in nursing required; bachelor's preferred.
• Minimum 1 year RN experience; home health experience preferred.

Responsabilidades

• Oversee the activities of the multi-disciplinary team that provides home care to the patient.
• Monitor therapeutic services through written and verbal communication with all disciplines.

• Shift Hours: 8:00 AM - 5:00 PM

The Home Health Field, RN provides hands‑on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include :

• Oversee the activities of the multi-disciplinary team that provides care to the patient
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
• Ensure that all visits scheduled and provided are authorized and covered by physician orders
• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits
• Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor

Required Certifications and Licensures:

• BLS (Basic Life Support)
• RN (Registered Nurse) License
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

Education:

• Required Associate's or Diploma Nursing
• Preferred Bachelors Nursing

Experience:

• Required 1 year Registered Nurse
• Preferred 1 year Home Care
