Job summary

Residential Home Health and Hospice is seeking a Home Health Consultant to partner with field staff and leaders to support patients with high-quality care. The role emphasizes building referral relationships, educating providers on Home Health, Journey, Palliative, and Hospice offerings, and coordinating with marketing and clinical management to meet capacity while maintaining care standards.

• Actively prospect for new referral sources based on the Agency’s scope of service.
• Work closely with Marketing, Agency Administrator and Clinical Management to ensure that the demand for clinical care is aligned with our capacity and at no time compromises the Agency’s ability to provide the best quality care for our patients.
• Educate referral sources on the Home Health, Journey, Palliative, and Hospice offering.

What you’ll do in this role:

• Maintain relationships with all existing referral sources.

We are looking for compassionate Home Health Consultant with:
