Job summary

Responsibilities

• Deliver compassionate, patient-centered care in the home health and hospice setting.
• Provide hands-on care for activities of daily living and report changes in condition.
• Educate patients and caregivers on care plans and daily routines.
• Maintain accurate, timely documentation in the EMR and coordinate with the care team.
• Travel to patient homes within the assigned territory.
