Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Valid driver’s license and ability to spend ~20% of workday driving to patient locations

Responsibilities

• Deliver compassionate, patient-centered care in the home health/hospice setting
• Assist with activities of daily living (ADLs) and monitor patient condition
• Educate and support patients and caregivers on care plans and daily routines
• Document accurately and timely in the EMR; communicate with the care team
• Work independently while collaborating with an interdisciplinary team
• Travel to patient homes within assigned territory
