Job summary

Premier Home Health Care is seeking a Home Health CNA to deliver patient-centered care in homes across Denver, CO. You will perform tasks per the plan of care, assist with activities of daily living, and support patient independence with hands-on nursing assistance.

This full-time role offers a flexible schedule, on-the-road work, and a benefits package including 401(k) with matching, dental, vision, and paid time off to support clinicians delivering high-quality care.

Qualifications

• Complete a State approved CNA Training program and has an active CNA certification in Colorado
• CPR Certification
• Pass a background check
• Knowledge of state and federal home health regulations
• Good clinical judgment, strong interpersonal skills, dexterity, and compassion

Responsibilities

• Provides services that are ordered in the plan of care and that the aide is permitted to perform under State law.
• Performs hands-on personal care.
• Performs simple procedures as an extension of the therapy or nursing services as assigned.
• Assistance in ambulation or exercises as assigned.
• Understands and adheres to established Agency policies and procedures.
• Completes appropriate visit records in a timely manner as per Agency policy.
• Reports changes in the patient's condition and needs to the appropriate personnel.
• Performs household services essential to health care in the home as assigned.
• Attends in-service and continuing education programs as scheduled and necessary.
• Attends staff meetings and patient care conferences as scheduled.
• Follows Agency policy for cleaning equipment between patient uses.
• Completes other tasks as assigned by the RN on the Plan of Care.
