Premier Home Health Care Inc. is seeking a Home Health CNA to deliver one-on-one patient care in the Denver area. You will follow the plan of care, perform hands-on personal care, assist with mobility, and document visits accurately.

Qualifications

• Completion of a state approved CNA training program and active Colorado CNA certification.
• CPR certification.
• Pass a background check.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide services ordered in the plan of care that the CNA is permitted to perform under state law.
• Perform hands-on personal care.
• Assist with ambulation or exercises as assigned.
• Complete visit records in a timely manner per agency policy.
• Report changes in patient status to appropriate personnel.
• Perform household services essential to health care in the home.
