Job summary

Elara Caring in Mentor, OH is seeking a Clinical Team Manager to oversee patient care services, staff assignments, referrals, and care coordination to meet patient needs.

You will plan Home Health services with families and physicians, ensure timely start of care, review care plans, and coach the direct care team while partnering with the Branch Director to monitor performance metrics and ensure regulatory compliance.

Qualifications

• Associate degree in a Nursing related field; Bachelor preferred.
• 2 years of home care experience in Home Health or Hospice environment.
• 1 year of supervisory experience required.
• Current, unrestricted RN license valid for the state of work.
• Ability to become proficient in enterprise applications such as Homecare Homebase, Workday, and Microsoft Office.

Responsibilities

• Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are met.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and ongoing compliance with applicable regulatory guidelines.
• Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
• Conduct regular OASIS reviews to support accurate documentation and quality patient care.
• Provide ongoing guidance, coaching, and support to direct care team members and assist with interviewing and hiring clinical staff.
• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.
