Job summary

Elara Caring is seeking an experienced RN Clinical Team Manager for our Lafayette, IN area to lead home health patient care services and oversee clinical operations. This role emphasizes coordination of care, supervision of direct care staff, and ongoing improvement of patient outcomes.

You will report to the Administrator or Branch Director and support field operations, ensuring compliance with regulatory guidelines while delivering compassionate, high-quality care to patients in their homes.

Qualifications

• Associate degree in nursing required and RN license active in Indiana.
• Minimum 2 years' home care nursing experience in Home Health or Hospice.
• 1 year of supervisory experience preferred.

Responsibilities

• Provides oversight of patient care services and personnel, including assignments and plan of care development.
• Plans, directs, and participates in delivery of home health services and coordination with providers.
• Ensures timely start of patient care within regulatory guidelines.
• Coordinates orders, staff assignments, and communications among care team, patient, family, and physicians.
• Reviews and approves plan of care and evaluates proposed changes for clinical appropriateness.
• Integrates orders from physicians and ensures physician approval.
• Conducts regular OASIS reviews for all patients.
• Meets with direct care team members for guidance and updates; participates in hiring decisions.
• Communicates performance metrics with Branch Director/Administrator; available for field work as needed.
• Performs other duties/projects as assigned.
