Job summary

Elara Caring is seeking a Clinical Team Manager to lead patient care services and staff in a home health setting. You will oversee care coordination, referrals, and the care plan, ensuring timely, regulatory-compliant service delivery and collaboration with physicians and the care team.

The ideal candidate has 2+ years of RN home health experience and 1+ year of supervisory experience, with strong proficiency in Homecare Homebase and Workday.

Qualifications

• Associate degree in Nursing required.
• 2 years home care experience as an RN in home health or hospice.
• 1 year supervisory experience preferred.
• Current, unrestricted RN license valid for the state of work.

Responsibilities

• Oversee patient care services and personnel; assign patients, coordinate care and referrals.
• Direct delivery of home health services and coordination with providers and facilities.
• Ensure timely start of patient care within regulatory guidelines.
• Process physician orders, staff assignments, and communications among care team, patient and family.
