Qualifications

• 2 years home care RN experience in a Home Health or Hospice environment.
• 1 year supervisory experience preferred.
• Current, unrestricted RN license valid for the state of work.

Responsibilities

• Oversee patient care services and personnel, including assignments, coordination of care, referrals, and individualized care plans.
• Direct delivery of home health services and coordination with providers, vendors, or facilities.
• Ensure timely start of patient care within regulatory guidelines.
• Process and coordinate physician orders, staff assignments, and communications among care team, patient, family, and physician.
• Review and approve plan of care and evaluate proposed changes for clinical appropriateness.
• Integrate orders from all physicians involved in the plan of care and obtain approvals.
• Conduct regular OASIS reviews for all patients.
• Lead regular meetings with direct care team to guide and inform on issues or changes; initiate hiring and interviews.
• Report key performance metrics to Branch Director or Administrator.
• Remain available for field work; handle high-stress environment and duties.
