Compensation

• Salary: Approximately $90,000+ (based on experience)
• Sign-On Bonus: $5,000

What You'll Do

• Oversee patient care across the full continuum: admissions, recertifications, routine visits, transfers, and discharge planning
• Provide clinical leadership, mentorship, training, and support to field clinicians
• Collaborate closely with case managers and interdisciplinary teams to ensure optimal outcomes
• Balance administrative oversight with hands‑on clinical support, including occasional patient visits
• Help drive quality, compliance, and best practices (including OASIS accuracy)
• Adapt to a dynamic environment with a proactive, solutions‑focused mindset

Requirements

• Active RN license
• Proven experience in home health and OASIS assessments (required)
• Leadership experience in a supervisory or case management role preferred
• Strong clinical judgment and the ability to work both independently and collaboratively
• Excellent communication skills and a team‑first attitude
