Job summary
Boost Home Healthcare - Southeast Michigan is seeking a Clinical Manager (RN) to oversee home health services and supervise staff in their patients’ residences. You will ensure care plans align with physician orders, manage performance Evaluations and staffing efficiency, and coordinate interdisciplinary care to achieve optimal outcomes.
Ideal candidates have an RN license in Michigan, 2+ years in community/home health, CPR certification, and experience with EMR.
Qualifications
- RN license in the state of operation.
- Two years RN experience, with two in community health/home health.
- CPR certification while employed in a clinical role.
- One to two years supervising clinicians in a Home Health Agency environment.
- Experience using Electronic Medical Records (EMR).
- Access to reliable transportation.
- Strong understanding of Medicare Conditions of Participation and payment mechanisms.
Responsibilities
- Provide direct clinical supervision, evaluations and disciplinary action of all assigned staff.
- Perform in-home supervisory visits with the team.
- Monitor staff assignments, caseloads and team productivity.
- Promote efficient utilization of resources to achieve clinical outcomes.
- Support coordination of interdisciplinary care.
- Develop and maintain patient tracking and financial forecasting.
- Provide census, staffing and productivity reports to Administrator.
