Job summary

Adoration Health in Anmoore, WV is seeking a Home Health Clinical Manager to lead a team of clinicians delivering patient-centered care at home. You will oversee care plans, ensure compliance, mentor staff, and partner with leadership to drive quality and outcomes.

Qualifications

• Bachelor's degree in Nursing preferred.
• Licensed Registered Nurse (RN) in good standing in the state of practice.
• Minimum of two years clinical experience and at least one year in a related management/supervisory role in home health care.
• Current CPR certification.
• Ability to communicate effectively with all levels of personnel.

Responsibilities

• Conducts/participates in quarterly quality improvement meetings and oversees improvement plans.
• Collaborates with Branch Director on quality improvement activities and corrective actions.
• Oversees surveys and audits related to quality of care and KPIs with operations leadership.
• Completes documentation and chart reviews to identify variances and areas for performance improvement.
• Holds case conferences to ensure oversight of care and coordination of services.
• Facilitates development of performance improvement action plans based on data (surveys, QAPI, chart reviews).
• Ensures staff evaluations are fair and timely per company procedures.
