Job summary

Adoration Home Health, an Adoration Health affiliate, seeks a Home Health Clinical Manager in Anmoore, WV to lead a team of clinicians and deliver high-quality, patient-centered care. You will oversee care plans, ensure regulatory compliance, and mentor staff in a supportive leadership environment.

You will collaborate with the Branch Director, drive quality improvement initiatives, and maintain strong relationships with patients and families while ensuring efficient operations and patient

Qualifications

• Bachelor's Degree preferred in Nursing
• RN license in good standing and currently licensed in state of practice
• Minimum two years clinical experience and at least one year in a related management role in home health care
• Current CPR certification
• Ability to use company documentation systems
• Strong communication (verbal and written) with all levels of personnel
• Ability to work independently as well as part of a team
• Capable of handling highly confidential information

Responsibilities

• Oversee quarterly quality improvement meetings and implement improvement plans
• Collaborate with Branch Director on quality improvement activities and corrective actions
• Oversee audits and surveys related to quality of care and KPIs
• Complete documentation and chart reviews to identify variances and opportunities for improvement
• Lead case conferences to ensure care coordination and standards are met
• Drive performance improvement actions through data analysis and coaching
• Ensure fair and timely staff evaluations per company procedures
