Job summary

Compassus is seeking a Home Health Director Clinical Services (RN) to supervise patient and family care per plan of care and manage the Interdisciplinary Team (IDT). The role includes scheduling, mentoring, and ensuring compliance with state and federal regulations, while coordinating with the Primary Care Physician and IDT members.

You will lead IDT meetings and oversee the EMR documentation workflow, maintaining high standards of quality and continuity of care in a Home Health setting.

Qualifications

• RN licensed in the state of employment.
• 1 year of home health care experience preferred.
• 1 year of supervisory experience preferred.
• Valid driver’s license and auto liability insurance.
• Current CPR certification.

Responsibilities

• Supervise patient and family care per plan of care and ensure quality.
• Direct and manage Interdisciplinary Team (IDT) including scheduling and mentoring.
• Oversee consultative process between PCP and IDT members.
• Lead IDT meetings and ensure regulatory compliance.
• Process EMR documentation workflow as needed.
