Qualifications

Responsibilities

• Supervise patient and family care per plan of care, ensuring quality and continuity.
• Direct and manage Interdisciplinary Team (IDT) scheduling and mentoring.
• Oversee IDT-physician consults and ensure regulatory compliance.
• Lead IDT meetings and manage EMR documentation workflow.
