AlphaCare Home Health Corp is seeking a Clinical Coordinator and Quality Assurance Nurse to improve quality of care, optimize clinical performance, and mentor staff across operations. The role partners with Clinical Managers, QA, Operations, Schedulers, and Intake to align patient outcomes with standards.

Ideal candidates have strong clinical and data-driven skills, EMR proficiency, and the ability to coach clinicians while supporting regulatory compliance and performance improvement initiatives.

Qualifications

• Active RN license in California (required).
• 3–5 years of clinical experience in home health, quality review, or care management.
• Experience in utilization review, quality assurance, or regulatory compliance preferred.
• Excellent communication, analytical, and teaching skills.
• Proficient in EMR systems and data reporting tools.
• Strong ability to coach and mentor clinicians while supporting operational excellence.

Responsibilities

• Quality Assurance & Regulatory Compliance: participate in audits and reviews to uphold standards.
• Clinical Utilization & Performance Oversight: review visit utilization and optimize for patient outcomes and payer compliance.
• Readmission and Outcome Management: track readmission data and develop preventive strategies.
• Clinical Support: provide real-time clinical guidance and case consultation to field staff.
• Education, Coaching & Field Mentorship: deliver training and orient new clinicians.
• Perform other related duties as assigned.
