Job summary

Accurate Home Care is seeking a Full Time Clinical Coordinator to support our home health team at the Otsego Office. This role emphasizes coordination of referrals, communication with nursing and physician offices, and organizing scheduling and patient records to ensure smooth care delivery.

Qualifications

• Licensed Practical Nurse license in Minnesota is preferred but not required.
• Excellent oral and written communication skills in English.
• Two years of home care experience preferred.

Responsibilities

• Assist with coordination of the patient referral/intake process.
• Communicate with nursing care team and physician offices.
• Organize multiple facets of care delivery including scheduling, communication, and management of patient medical records.
