Job summary

Elara Caring is hiring a Certified Nurse Aide to provide at-home patient care under the supervision of a Registered Nurse. You will assist with daily activities, monitor vital signs, and support safe mobility in patients' homes.

Responsibilities include following care plans, reporting changes to the team, and maintaining documentation while ensuring privacy and infection control standards.

Qualifications

• Current State Home Health Aide or Certified Nursing Assistant certification.
• Ability to lift up to 50 pounds and assist with transfers.
• Valid driver’s license or reliable transportation to patient homes.

Responsibilities

• Provide hands-on personal care under RN direction.
• Assist patients with bathing, dressing, grooming, toileting, and daily activities.
• Take and record vital signs: temperature, pulse, respirations, BP.
• Help patients walk, transfer, and use mobility equipment.
• Follow each patient’s individualized care plan.
• Monitor and report changes to the care team.
• Maintain a clean, safe home environment.
• Remind patients to take medications as directed.
• Respond to emergencies and follow safety/infection control guidelines.
• Maintain documentation and protect patient privacy.

As a Certified Nurse Aide, you’ll contribute to our success in the following ways:

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient's individualized care plan.
• Monitor and report changes in a patient's condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation, protect patient privacy, and participate in required training.

What is Required?

• Current State certificate as a Home Health Aide certification or Certified Nursing Aide certification
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
