Job summary

Vitruvian Health is seeking a Case Manager RN to provide comprehensive nursing care for geriatric patients in home settings. You will develop, implement, and adjust care plans under physician collaboration, ensuring high-quality, age-appropriate care.

The role requires Georgia RN licensure, BLS CPR, and the ability to supervise Home Health Aide and LPN while coordinating multiple disciplines. Part-time, travel within the Hamilton Medical Center service area is expected.

Qualifications

• Graduate of accredited nursing program (Diploma/ASN/BSN).
• Current Georgia or multi-state RN license; BLS CPR required.
• Experience in home health or geriatric care preferred.

Responsibilities

• Provide comprehensive nursing care using the nursing process for geriatric patients at home.
• Develop, implement, and evaluate the patient’s care plan with physician collaboration.
• Direct and supervise Home Health Aide and LPN; coordinate other disciplines.
• Maintain patient records meeting state documentation requirements.
