Job summary

PruittHealth Premier is seeking a Nurse Case Manager to oversee clinical care management for community-based DSNP members in Georgia. You will work remotely with limited in-home visits, coordinating care across providers and social supports to optimize health outcomes and cost-effective resource use.

Responsibilities include assessments, care planning, and collaboration with plan navigators, pharmacists, and interdisciplinary teams to ensure continuity of care and patient empowerment.

Qualifications

• Advanced Nursing Diploma and/or college degree in nursing.
• Minimum two years direct clinical care experience.
• Licensed Registered Nurse in Georgia.
• CCM certification preferred.
• Ability to work remotely and manage caseload.

Responsibilities

• Conduct in-home and telephonic assessment and data collection, documenting findings.
• Develop a case management care plan based on history and clinical review.
• Monitor needs and adjust the plan of care as changes occur.
• Coordinate with Plan Care Navigator and discharge planning.
• Educate patients and provide emotional support.
• Ensure medication therapy management and compliance with pharmacist.
• Create quality CM reports documenting results of CM plan interventions.
• Coordinate benefits and with PCP/specialists for continuity of care.
