Job summary

Hill Country Preferred Senior Care is seeking a Home Health RN/LVN Case Manager in Mason, TX. You will provide skilled nursing care, manage patient plans of care, and coordinate with physicians and care teams to ensure quality home health services.

The role requires active Texas RN or LVN licensure, 1+ year home health experience, strong clinical skills, and excellent communication. OT-friendly schedule is Mon-Fri, 8am-5pm with driving responsibilities.

Qualifications

• Current Texas RN license (or LVN)
• Minimum 1 year home health care experience preferred
• Strong clinical and disease management knowledge
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and time management skills
• Valid driver's license and reliable transportation

Responsibilities

• Performs skilled nursing services under supervision
• Observes and reports patient conditions
• Maintains and updates clinical records weekly
• Communicates changes in patient status to physicians
• Assists with referrals, scheduling, and coordination
• Oversees team compliance and educates staff
• Participates in weekly staff case conferences
• Manages insurance authorizations and documentation
