We are seeking a dedicated and compassionate Case Manager for our Home Health department to join our team. In this role, you will play a crucial part in coordinating care for patients within their homes, ensuring they receive the highest quality of care and support during their recovery process. As a Case Manager, you will work closely with patients, families, healthcare providers, and community resources to develop individualized care plans tailored to meet the unique needs of each patient. You will be responsible for conducting comprehensive assessments, monitoring patient progress, and facilitating communication among all stakeholders involved in a patient’s care.
Responsibilities
Conduct comprehensive assessments of patients' needs and health status.
Develop, implement, and oversee individualized care plans in collaboration with patients and healthcare providers.
Monitor patient progress and adjust care plans as necessary to meet evolving needs.
Coordinate and facilitate communication among patients, families, healthcare team members, and community resources.
Provide education to patients and their families regarding health management and available services.
Advocate for patients by addressing barriers to care and ensuring they receive necessary resources.
Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations.
Requirements
Bachelor's degree in Nursing, Social Work, or a related field; relevant certification .
Current licensure or certification as required by the state.
Minimum of 2 years experience in case management or home health care.
Strong knowledge of community resources and healthcare services.
Excellent communication and interpersonal skills to effectively interact with patients and healthcare teams.
Ability to assess patient needs and create effective care plans.
Proficient in electronic medical record (EMR) systems and Microsoft Office Suite.
