Job summary

BrightCare HomeCare is seeking a compassionate Home Health Caregiver to support clients in their homes across Gretna, Louisiana. You will provide direct patient care, assist with activities of daily living, monitor health status, and help implement individualized care plans to promote comfort and independence for each client.

Responsibilities

• Provide direct patient care, including assistance with ADLs.

• Implement and update patient care plans with the healthcare team.
• Assist with medication administration, wound care, and other treatments.
• Maintain a clean and safe home environment (laundry, housekeeping, meals).
• Communicate effectively with patients, families, and healthcare team.

As a Home Health Caregiver with BrightCare HomeCare, you will play a vital role in providing compassionate and personalized care to individuals in their homes. Responsibilities

• Provide direct patient care, including assistance with activities of daily living (ADLs), such as bathing, dressing, grooming, and toileting
• Maintain accurate and detailed records of patient care and progress
• Implement and update patient care plans in collaboration with the healthcare team
• Assist with medication administration, wound care, and other medical treatments as needed
• Maintain a clean and safe environment, including laundry, housekeeping, and meal preparation
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Collaborate with our multidisciplinary team to develop and implement individualized care plans
• Participate in continuous quality improvement initiatives to enhance patient care and outcomes

Requirements

Work Environment

• Work in clients' homes in the Gretna, Louisiana area
• Must be able to travel to and from client homes, as needed
• Ability to work varied shifts, including days, evenings, and weekends
