Job summary

ATEX Healthcare in McAllen, TX seeks an RN or LVN Patient Care Coordinator to ensure seamless care transitions for home health and hospice patients. You will coordinate the plan of care, supervise staff, and ensure regulatory compliance.

Qualifications

• RN or LVN licensure in the state employed.
• Bachelor’s degree in Nursing preferred.
• At least two years nursing experience with home health and supervisory experience.
• Demonstrates good communication and management skills.
• Must be a licensed driver with a reliable vehicle.

Responsibilities

• Coordinate establishment and implementation of patient care plans and communicate policies.
• Monitor delivery of patient care and coordinate with support services.
• Perform on-site evaluation and supervision of field staff.
• Assist with Performance Improvement planning and documentation reviews.
• Coordinate 24/7 nursing coverage and ensure regulatory compliance.
• Maintain records of clinical meetings and education programs.
